PURPOSE:
Sometimes a guest may not be transferred home but may go on to further respite facility or hospital. Therefore, it is important to inform the receiving place about the individuals care needs
PROCEDURE:
- A hospital/transfer pack is available and pre-populated on PCS in the event of a guest being transferred.
- A transfer pack should be printed and sent to inform the establishment about the individuals care needs.
- Any other information which may be appropriate can be photocopied and sent with the guest also, (e.g. copy of medication, and if on any specialised care, for example PEG)
- It is important to confirm that the next of kin is aware of the transfer, so a phone call to them would be have to be made.
- Information to be passed on to the guest’s social worker, if applicable.
- The rest of the procedure is as per the guest being discharged (see discharge procedure)
