Pre-admission telephone call – Assessing the Needs of our Guests Procedure Notes

Stage 6 – Pre-admission telephone call

A member of the assessment team will call every guest no earlier than 7-10 working days before the date of their holiday. The purpose of this call is to:

  • Confirm there have been no changes to the guests care needs since the date of their formal nurse assessment
  • Allocate an arrival time and document travel arrangements / station collections
  • Remind each guest of what they need to bring with them
  • Give each guest another opportunity to ask any questions they may have

Procedure

  • Gather the guest care plans for the next short break which the calls relate
  • Complete the pre-admission call log with each guest name so that we can see at a glance who has been called, when and by whom. These forms are then stored in the yellow pre-admission call folder
  • Identify who the recipient of the call should be – this is documented on the nurse assessment
  • Call the guest or appropriate representative and follow the questions to be asked on the ‘Pre-admission call checklist’. If the call is unable to be completed then we must document our attempts to call; there is space on the checklist to do this. On completion this form should be signed and dated and filed in the guest care plan in the pre-admission documentation
  • If the guest has had any changes to their care needs since the date of their nurse assessment then the call must be referred back to a nurse for further assessment. We should advise that this may impact on the guest’s dependency score.
  • Update the arrival times, method of transport and who the guest is travelling with on the arrival time form. This form is distributed to all care staff, catering and reception on the outgoing Thursday of the week preceding the start of a new holiday
  • Advise Physio/OT if any Leuchie equipment is required on arrival of the guest

On completion of the pre-admission calls all files should be taken to the nurse’s office on the 1st floor
