Nurse Assessment – Assessing the Needs of our Guests Procedure Notes

Stage 3 – Nurse Assessment

Before any short break is confirmed a nurse must assess each guest to determine their suitability for a placement at Leuchie. This is initiated once:

  • Our Pre-admission self-assessment (PASA) has been completed and returned by the guest
  • A local authority single shared assessment (if appropriate) has been received.

It is the responsibility of the assessment team administrators to obtain this information before a nurse is asked to conduct an assessment. The assessment should be completed within 7 days of the nurse receiving all the relevant paperwork.

Procedure

  • The PASA and single shared assessment (where relevant) is reviewed and the guest or appropriate representative is called by an RN, if required, to discuss and validate the information provided.
  • The assessment process is guided by the RN assessment checklist which must be followed.  Any emotional, social or behavioural concerns should be discussed more widely to ensure needs can be met.
  • If further information is required then the nurse will contact other external health professionals and update details on the RN assessment checklist
  • Once the information that the nurse considers necessary has been obtained then the dependency score of the guest is determined. This indicates whether or not we are able to meet the needs of the guest and guides our overall dependency score for each holiday. This informs not only if we can accept a guest but also in which holiday. This information is noted in the dependency score totals spreadsheet
  • An appropriate bedroom is allocated for the guest at this stage and allocated on the dependency score spreadsheet
  • The nurse’s decision as to whether or not we can meet the needs of a potential guest is recorded on the RN assessment checklist and the pre-admission booking form. If holiday dates need to be changed due to dependency levels this would also be documented.
  • This paperwork is then passed back to the booking team who will continue the process – see procedure (4) confirming or declining a booking.
