Stage 8 – Continual risk assessment throughout stay
In May 2019 Leuchie House purchased the new Person-centred software. An electronic method of recording care via hand held devices.
Much of the information about a guest is entered into PCS via the Monitor by Nursing admin prior to the guest’s arrival and only updated where necessary, on arrival.
Information should be used from the most recent source such as social work department single shared assessment or preadmission shared assessment (PASA) to complete the individuals.
Before a guest arrives, risk appropriate assessments should be added and any available information on the guest should be added to the PCS care plan
Information on a guest should be reviewed throughout their stay and the care plan on PCS updated.
Changes in dependency should be communicated with the guest/family prior to discharge and reasons why.
If a guest is deemed unsuitable following their stay this must be communicated to the guest/family prior to discharge along with the reasons why.
It is expected that all members of the Leuchie team will treat all guests, colleagues and volunteers and those who come in contact with Leuchie with respect at all times and they in turn expect to be treated with respect by all guests and volunteers.
‘From time-to-time Leuchie’ s team, guests or carers identify changes in a guest’s needs, wants, health or behaviours between the booking assessment and the start of or during their short break. We recognise that this does occur and Leuchie will always endeavour to meet those needs. Occasionally however, such changes are so significant that they can be difficult to accommodate in our group care setting and with the available resources and team. In these very rare circumstances Leuchie’ s actions will always be guided by its desire to achieve a positive outcome for all, to provide a good quality and safe service and to respect the dignity and rights of all guests and colleagues.
Leuchie recognises that involving guests in decisions is essential to a positive outcome and potentially complex, emotional and at times difficult for all parties. Careful thought will therefore be given to the timing of the discussions, to the opportunity for guests to have an advocate present and to presenting information clearly and in a manner which the guest can understand.
In these rare circumstances, Leuchie will meet with the guest to raise any concerns and seek a positive solution.
If following such a meeting Leuchie identifies that continuation of the current break or offer of a future break is in question, it will seek further and separate meetings to outline its rationale and seek the views of the guest towards a positive outcome.
Leuchie may determine, after careful review that it is not able to meet the guest’s needs on future breaks. It will give careful thought to how to present this decision in line with this policy.
In the rarest of circumstances, and only when provision of care cannot safely continue, Leuchie will ensure that no cessation of service takes place before sufficient, alternative care arrangements are put in place.
Guests and their carers have the right to raise a complaint at any time with the Care Inspectorate and to be supported to raise that complaint. This will be noted in any meeting identified above.
